How Can I Take the Health Survey?

Taking the health survey is simple and only takes a few minutes of your time. You can take it in two different ways: 

  1. Online

    You may take the health survey via the myMDwise member portal.

    To begin, log in to your account. Please note that it may take up to two weeks to gain access to the member portal after you become an MDwise member. If you’re unable to log in, please try again after the initial two-week period has passed.

  2. On the Phone

    You may take the survey via the phone. To get started, call 877-822-7193. A representative will assist you.
